A contractor faces fines of more than $48,000 for fall hazards on a job site. The contractor was cited by OSHA for five repeat and two serious violations. Four employees were found not wearing fall protection, and three of those workers (who were on a roof) had not received fall protection training. The one employee on a scaffold had not been trained to identify scaffolding hazards.

Worker Loses Finger in Machine Accident
A manufacturer faces OSHA fines of $76,000 for two repeated, four serious and one other-than-serious safety violation.
OSHA began investigating the manufacturer after a worker’s finger was partially amputated when it was pinched while unjamming a reel on a wood veneer machine.
OSHA inspectors noted that the manufacturer didn’t: guard operating parts of machinery, implement adequate machine safety procedures for clearing jams, install railings on open side platforms and walkways, and train workers in machine safety procedures.

Retailer Cited for Failing to Keep Workers Safe
A retailer faces proposed fines of $163,000 for safety violations that put workers at risk.
One violation was a blocked exit, which would have prevented employees from evacuating the store in the event of an emergency.
An OSHA investigation also revealed the following: workers exposed to fall hazards because stairs lacked railings; an exit sign posted at a door that wasn’t an exit; an unlit exit way and non-working emergency exit lighting; unlabeled circuit breakers; electrical panels blocked by miscellaneous material; and workers were not trained in fire extinguisher use.
